All forms are to be submitted to [email protected]. If you have questions regarding the form, you may schedule an appointment with an Academic Counselor.

College Honors Forms

• Upper Division Honors Application

Students must submit both the Upper Division Honors Application to [email protected] prior to the start of their Junior year. Students must receive Department approval prior to submitting the Upper Division Honors Application to the College of Science. 

Second Discipline Elective Forms

• Chemistry Second Discipline Electives (Catalog years 2022 or earlier)
• Mathematics Second Discipline Electives (Effective: 2016-17)
• Physics Second Discipline Electives

Students must receive Department approval prior to submitting the Secondary Discipline forms to the College of Science.

• College of Science Admission Application
• Change/Declaration of Major/Minor
• Grade Exclusion
• Request to Change Final Exam (3 exams in 24-hour period)
• Senior Research Thesis Honors Application
• Graduation Date Declare or Change Form
